The Navigators Voluntary Dental Plans

Dental care is an important part of your overall health. The Navigators offers a voluntary dental plan through UHC. As a member, you may receive discounted rates, and preventive care is covered at 100% in the UHC network. To keep you healthy beyond your dental care, the plan also provides oral cancer screenings and extra dental visits during pregnancy and the three months after birth.

*Benefits with an asterisk ( * ) require that the deductible be met before the Plan begins to pay.

If you use an out-of-network provider, you will be responsible for any charges above the maximum allowed amount.

Plan Documents

Using Participating Providers: While you are free to use any provider of your choice, you will maximize your benefits and reduce your out-of-pocket costs if you use providers who participate in the UHC network. You can find in-network dentists by visiting myuhc.com.

Dental ID Cards: UHC does not provide dental ID cards. When you visit your dentist, provide them with your group number (# 932695) and your Social Security number, and they will contact UHC to verify your coverage.
